Three easy steps to your new Passport:                                                                        

Step 1

Print each document that you will need. Then complete each form using a blue-pen, DS-11 form must be signed in front of the Acceptance Agent. When you have completed all forms call us at (646)-246-2181, and we will go over the process with you.

Step 2

Take all completed forms and necessary documents to your local Acceptance Agent. The Acceptance Agent will give you a sealed envelope (do not open it). Click here to find the Acceptance Agent nearest you.  These agents are typically located at your local Post Office.

Step 3

Use our Online Order Form to pay our expedite fees.  Then, ship your documents using our pre-paid FedEx option or the carrier of your choice.  (FedEx pre-paid shipping can be chosen when using our Online Order Form)

You Need a New Passport If:
  • You have never had a passport.
  • Your passport was issued more than 15 years ago.
  • Your passport was issued before you turned 16 years old.
  •  You are applying for a minor under the age of 16 and you are their legal guardian.
